Who qualifies for OLTL / Community HealthChoices services?
Adults 21 and older who are enrolled in Medicaid and meet the clinical eligibility criteria for nursing facility level of care may qualify for Community HealthChoices (CHC) waiver services.

What is the difference between an agency and a registry?
Under the agency model, Galeron employs and manages your caregiver. Under the registry model, you may select an independent caregiver. Both options are available depending on your waiver program.
Can I choose my own caregiver?
Yes. Depending on your waiver program and service type, you may have the option to select a family member or trusted individual as your caregiver through the registry model.
